New Patient Information

Taking the first step toward mental health support and therapy one of the most important things you can do for yourself or your loved one. We’ve made the process as simple and welcoming as possible — because getting started shouldn’t feel like another hurdle.

How to Become a New Patient

Getting connected with the right clinician is easy. Simply complete our secure online form and our Intake Coordinator will follow up with you to discuss your needs, answer any questions, and help match you with the right clinician.

What to Expect

We want your first experience with us to feel as smooth and comfortable as possible. Here’s a simple overview of what the process looks like:

  1. Submit your request

Complete our secure New Patient Form and we’ll begin our process.  Our Billing Manager will verify mental health insurance benefits.  Next, our Intake Coordinator will follow up to answer any additional questions.  If we are able to match you with an available and appropriate clinician, our Intake Coordinator will reach out with next steps.

We are committed to making sure everyone who reaches out feels heard. If for any reason we are unable to accommodate your request, our Intake Coordinator will let you know promptly and, whenever possible, offer referrals to other resources who can help.

  2. Complete your intake paperwork

Before your first appointment, you’ll receive a link to create a secure Patient Portal account andcomplete your intake forms online at your own pace. This helps your clinician come prepared and makes the most of your time together.  These forms must be completed prior to scheduling an intake appointment.

  3. Choose how you meet

We offer counseling and medication management appointments both in-person in Madison, WI and via telehealth, so you can choose the format that works best for your schedule, comfort level, and location.  Telehealth services can only be provided for patients when they are physically located in Wisconsin.

  4. Your first appointment

Your clinician will take time to get to know you, understand what brings you in, and begin exploring what care might look like going forward. There’s no pressure to have everything figured out — that’s what we’re here for.

  5. Building your care plan

Together with your clinician, you’ll begin shaping a personalized mental health treatment plan that fits your goals, your life, and your pace.

Insurance & Payment

We believe cost should never be a barrier to getting the care you need. We are in-network with several major insurance plans — see below — and also offer self-pay options for those who prefer to pay out of pocket.

Insurance participation varies by clinician. Our team is happy to help you confirm coverage before your first appointment.

Don’t hesitate to reach out with any questions about coverage or cost before booking scheduling.

No Surprises Act & Good Faith Estimates

We are committed to transparent mental health care pricing and billing communication before services begin.

  • The No Surprises Act (effective January 1, 2022) is a federal law designed to protect patients from unexpected medical bills.

  • Who it applies to: Any patient who is uninsured or who chooses not to use their insurance for services (i.e., self-pay patients).

  • What a Good Faith Estimate is: Before beginning services, our practice will provide a written cost estimate that includes the expected costs of care — session fees, anticipated frequency, and duration — so patients can make informed financial decisions. A Good Faith Estimate is not a bill, a contract, a diagnosis, or a guarantee of final cost. It is a projection based on your anticipated needs and is meant to support informed decision-making about your care.

  • Key client rights under the law: If a final bill exceeds the Good Faith Estimate by $400 or more, patients have the right to dispute the charges. They may ask for a revised estimate at any time as their care needs change.

  • What it does NOT cover: GFEs are not required for patients actively using their insurance benefits, though we offer them to anyone who asks.
